Working Together Award winners!
With Healthwatch Lambeth we're very excited to announce that we've won a national award! We won the 'Local Healthwatch working together' award at the Healthwatch Network Awards.
This is what Healthwatch England had to say:
Healthwatch Lambeth and Healthwatch Southwark have been recognised for their work over the last 12 months helping to improve the experiences of local NHS patients and social care users.
Nominated against tough competition from across England in the 'Local Healthwatch Working Together' category, Healthwatch Lambeth and Healthwatch Southwark came out top for their work improving people's experiences when they leave hospital.
For their project 'Going home', they tracked people for up to three months after they were discharged to see if they were getting the care and attention they needed. They spoke to people directly, and to their carers.
They shared what they learnt with CCGs and others and, as a result, have been asked to keep looking into people's experiences and to make recommendations for how things could be better.
Not only have they learnt a huge amount about the impact they can make in partnership, but they're also making a real difference to people.
Judging the award, Chair of Healthwatch England, Jane Mordue, said:
- "These two Healthwatch set out with a clear aim of tackling some of the longstanding and previously intractable problems around the discharge of elderly and vulnerable patients across the two boroughs.
- "Using their expertise to help evaluate a series of new initiatives, such as 'discharge to assess' and 'step-down' schemes, they have successfully demonstrated the value of seeking out feedback from patients along their journey rather than at just one point.
- "The fact they have been asked by their commissioners to repeat their methodology for other pieces of work is a testament to the success of this approach."
