Sharing your care record to get you faster, safer, more effective support and treatment
Health and care organisations across south east London are working to improve your care by connecting two successful electronic care
Currently, two separate systems offer a secure way of sharing information electronically between local staff involved in your care. This means that not everyone uses the same system and information can only be shared in pockets of south east London.
So, for example, if you live in Lewisham but are receiving treatment from Guy's and St Thomas' NHS Foundation Trust – your records would not be immediately available to the staff looking after you or making the referral.
Our Healthier South East London is working to link these two systems together – connecting up some 15,000 care professionals across six boroughs to benefit approximately 2 million people. Only relevant information about you will be securely and instantly shared between the staff who need it.

Care and treatment you need can be given earlier and closer to home - this stops your condition getting any worse and avoids hospital admissions
Care is more joined up so you won't have to tell your story to lots of different professionals
Transfer of care between services is smoother
Reduces unnecessary appointments, tests and associated costs
Your confidential health records are available to care professionals via a secure online portal rather than replying post, fax and email
You can still express your views around how your information is shared and with who, by speaking to the person caring for you or your GP.
Information sharing across south east London is expected to launch in early 2018.
